In accordance with Texas House Bill 4224, Whitehead Chiropractic provides the following instructions for our patients regarding their health information and consumer rights.

- Step 1: Contact our Office. Please call us at 817-424-4044 to request a Medical Records Release Form.
- Step 2: Complete the Form. Return the signed form to our office in person or via secure email. You must provide your full legal name, date of birth, and the specific dates of service requested.
- Step 3: Processing. Per Texas law, we will fulfill your request within 15 business days of receiving a valid, signed authorization.
- Fees: A reasonable, cost-based fee may apply for paper copies or mailing.
