In accordance with Texas House Bill 4224, Whitehead Chiropractic provides the following instructions for our patients regarding their health information and consumer rights.

1. How to Request Your Medical Records
You have the right to access or receive copies of your healthcare records. To ensure your privacy, we follow a specific process for all requests:

• Step 1: Contact our Office. Please call us at 817-424-4044 to request a Medical Records Release Form.
• Step 2: Complete the Form. Return the signed form to our office in person or via secure email. You must provide your full legal name, date of birth, and the specific dates of service requested.
• Step 3: Processing. Per Texas law, we will fulfill your request within 15 business days of receiving a valid, signed authorization.
• Fees: A reasonable, cost-based fee may apply for paper copies or mailing.

 

2. Licensing & Disciplinary Authority

Whitehead Chiropractic is regulated by the Texas Board of Chiropractic Examiners (TBCE). If you have questions regarding professional standards or wish to verify a license, you may contact the board directly:

3. How to File a Consumer Complaint

We strive to provide the highest quality care. However, if you have a complaint regarding our practice, billing, or the handling of your Protected Health Information (PHI), you may file a report with the following agencies:
